The term documentary was first used in 1926 by filmmaker John Grierson as a term to describe films that document reality. Distinct from Factual Entertainment, pure documentary is both unscripted and seeks to record 'stories' as they happen with minimal editorial bias and interference. Well, thats the theory anyway.
